Domestic Homicide Review (DHR) – ‘Henry’, republished December 2025

Publication Statement on behalf of the MK Together Safeguarding Partnership

“This comprehensive review examined agency responses and support given to ‘Henry’, a resident of Milton Keynes, and his family prior to his death in April 2019. It highlights the devastating impacts of domestic abuse and the complex nature of depression and substance misuse. This review was originally published in 2024 and was temporarily withdrawn whilst further discussions with the family took place.

Colleagues across the police, health and social care have worked closely together to identify all possible learning from this review. Recommendations have been made, with an emphasis on additional communication and information sharing between agencies, and further consideration for the support needs of family members in such cases. These changes have been taken forward across the partnership.

The Safeguarding Partners will continue to support and challenge agencies to deliver the highest standards of safeguarding for adults in Milton Keynes.”
