Professionals from agencies, including police, health and social care organisations that work across Milton Keynes work together to review findings and implement learning from statutory reviews, supporting continuous improvement in safeguarding services.

“This review examined agency responses and support given to a family prior to the death of their child in February 2024. It highlights the risks of co-sleeping and the devastating and complex nature of sudden unexpected death in infancy.
Whilst it is recognised that significant and positive developments have already been made since 2024, colleagues across the police, health and social care have worked closely together to identify all possible learning from this review. Recommendations have been made, with an emphasis on how concerns surrounding potential issues are assessed and the way this information is shared between agencies.
Safeguarding Partners will continue to support and challenge agencies to deliver the highest standards of safeguarding support for children and young people in Milton Keynes.”

The review highlights the complexity of depression and bipolar disorder and identifies opportunities to strengthen how colleagues across police, health and social care work together to support individuals and their families.
While progress has already been made since 2024, the Safeguarding Partners will continue to support and challenge agencies to deliver the highest standards of safeguarding for adults in Milton Keynes.”
Domestic Homicide Review (DHR) – ‘Henry’, republished December 2025
This review was originally published in 2024 and was temporarily withdrawn whilst further discussions with the family took place.
Colleagues from police, health and social care worked closely to identify all possible learning from this review. Recommendations have been made, with an emphasis on additional communication and information sharing between agencies, and further consideration for the support needs of family members in such cases. These changes have been taken forward across the partnership.
The Safeguarding Partners will continue to support and challenge agencies to deliver the highest standards of safeguarding in Milton Keynes.”
